I had one of the other pill crushers and, as another reviewer pointed it out, the threads would jam unless I split the pills into smaller pieces first. That was annoying enough that I went looking for something better. The Ultra Pill Crusher is it. This one works so well I have no need to pre-split anything. It is well thought out and well manufactured. The threads are long enough so you can get them completely started before coming up against a whole pill and they are smooth enough to turn easily. You spin the top down until it comes up against the pill and then a little effort easily breaks the pill. What is especially clever is the shape of the pill cavity. Both the bottom and top pieces are slightly conical. This gives the top a bit of a point which immediately breaks the pill with very little effort. And since the bottom is also conical, any pill crumbs fall towards the center so when you can back off a bit and re-tighten the top, everything is converted to powder. My co-workers and I in my hospital unit all think the crusher works just fine and we like the grip you can get on the top because it isn't justround. Only drawback is that the base edges are kind of sharp as most of us hold it in our hands while we are crushing meds.I have a g-tube so all my meds have to be crushed to fit through the tubing. I have literally worn out pill crushers. I got this one thinking it would be easier on my hands to use since I take 40 pills a day. The top half is easy to grip but the bottom half hurts my hands to hold. The sharp edges/ridges on the bottom really cut into my hand. Why didn't the top design and materials get repeated on the bottom? Also, the internal action of the crusher doesn't crush the pills completely. The several times I have tried I have ended up pouring the pieces into my old worn out crusher to finish the job. I can't believe there isn't a crusher out there that works, is durable and is easy to grip. This one almost got it half right.
